ADMISSION REQUIREMENTS FOR MASTERS/LICENSURE STUDENTS
1. Applicants must complete two (2) applications with two(2) separate deadlines:
A.The University of Utah Application for Admission to the Masters/Licensure Program in the Department of Special Education
. Return it to the Department of Special Education.
B. The current University of Utah Graduate School Application for Admission to Graduate School. Return it to the Admissions Office, 250 Student Services Building. Be sure to read the instructions carefully, including information on the fee schedules.
(Deadline for receipt is 30 DAYS prior to Department deadline. The Graduate School will not process your application without the required fee and original copies of ALL transcripts).
2. Register for and complete the Graduate Record Examination (GRE). International Students must also take the TOEFL (including TWE) and either TSE or SPEAK. TEST SCORES MUST BE CURRENT (TAKEN WITHIN THE PAST FIVE YEARS). Refer to the information sheet regarding test administration, test dates, and related information. STUDENTS SHOULD SPECIFY ON THE GRE REGISTRATION FORM THAT THE TEST RESULTS BE SENT DIRECTLY TO THE DEPARTMENT OF SPECIAL EDUCATION.
* TEST SHOULD BE TAKEN AT LEAST TWO MONTHS BEFORE THE DEPARTMENTAL DEADLINE IN ORDER FOR THE TEST RESULTS TO BE IN BY THE DEADLINE. *
3. Provide a two-three page, double-spaced typewritten professional career statement. The statement will be used as part of the data considered for admission.
4. Send THREE (3) CURRENT RECOMMENDATIONS (must use University of Utah, Department of Special Education Recommendation Forms
) from persons capable of speaking authoritatively about your academic ability and teaching potential. THESE FORMS MUST BE MAILED OR HAND CARRIED BY THE WRITER DIRECTLY TO THE DEPARTMENT OF SPECIAL EDUCATION AT THE ADDRESS BELOW. These recommendation forms will not be accepted if hand-carried by the applicant. To assist these references, BE SURE YOU FILL IN THE DEADLINE DATE ON THE FORM. (Recommendations of relatives will not be accepted. )
5. Send copies of official transcripts from EACH University or College attended.
6. Send in items 1A and 2-5 to:

7. It is the responsibility of the applicant to ensure that ALL admissions materials, including recommendations, are received by the Academic Advisor before the deadline. Application materials, including Transcripts and Recommendations, will not be accepted beyond the final deadline for each specified admission period. The Department of Special Education is not responsible for any admissions materials sent to the wrong address. THREE WEEKS BEFORE THE DEADLINE, A LETTER WILL BE SENT NOTIFYING YOU (the applicant) WHAT IS STILL NEEDED TO COMPLETE YOUR APPLICATION FILE.
DOUBLE-CHECK WITH THE DEPARTMENT OF SPECIAL EDUCATION'S ACADEMIC ADVISOR BEFORE THE FINAL DEADLINE IF YOU NEED TO MAKE SURE YOUR FILE IS COMPLETE.
The Admissions Committee is able to accept only COMPLETE application files.
| NOTE: CRIMINAL CONVICTION: You should know that if you have been convicted of something other than minor traffic violations, this may be a bar to your being licensed as a teacher. If you have been so convicted, you should consult with either the Associate Dean for Professional Education of the Graduate School of Education, or the Utah Professional Practices Advisory Committee, c/o Executive Secretary, State Office of Education, before proceeding with your program in teacher education. |
FOLLOW-UP PROCEDURES
1. Completed admission files which meet the deadline will be reviewed by the Admissions Committee. WITHIN APPROXIMATELY THREE WEEKS AFTER THE POSTED FINAL DEADLINE, you will be notified by mail of the status of your application. The Committee recommends admission, denial, or no action (e.g. , if the file is incomplete) to the Graduate School. The actual decision is made by the Graduate School.
2. Admission files and the materials contained therein become the property of the Department of Special Education and will not be returned to the applicant. An incomplete file will be kept for one (1) year from the date of application, after which it will be destroyed.
3. DENIALS:
Questions or concerns about the basis for denial should be directed to the Chair of the Admissions Committee. Students who are denied admission may reapply to the department during a subsequent admissions period if they believe that new or updated materials will form the basis for admission to the department. They are encouraged to work with the Department Academic Advisor as they prepare their materials to make sure that their file is both complete and current.
4. APPEALS:
If a student wishes to appeal a decision denying admission to the Department, the student should write a letter to the Department Chair describing the basis for believing that the decision violated the Admissions Committee=s written standards and procedures. The Department Chair will review the decision for compliance with Committee standards and procedures. Based on the findings, the Chair will make a determination to uphold or reverse the decision.

CHECKLIST OF APPLICATION MATERIALS TO BE SUBMITTED:
Approximately three weeks prior to the deadline, you will be notified by mail concerning the status of your application file. Only complete application files will be accepted by the department=s Admissions Committee. These files must include:
